The End User shall be defined as the person submitting this form. The Provider shall be defined as deltaComm Development, Inc, a North Carolina Corporation, or its assignees.
End User agrees that submission of this form constitutes an agreement to pay for the products or services described on said form. Telix products carry a 30 day money-back guarantee. Any refunds will not include shipping charges -- once a shipping charge has been incurred, it is non-refundable. Unsolicited or unauthorized returns will not be accepted. All returns must carry an RMA number issued by Provider. Provider agrees to issue such an RMA within 30 days of the invoice date upon request, with no questions asked.
End User's credit card will be automatically debited for this order; by submitting this form containing credit card information, End User is agreeing to pay the charges as described in their agreement with their credit card issuer.
In the case of a dispute between End User and Provider regarding billing, End User agrees to contact Provider first for a resolution of the issue. End user agrees not to involve the card issuer or bank (i.e. to initiate a chargeback) unless a refund within the terms of this agreement is refused by the Provider.
